In order to become a buyer and access equipment pricing, you must fill out the form below. The annual cost is $200, but this year we are waiving the fee.
Before filling out this form, make sure to download the CRT-61 Certificate Form, print it, complete it, and sign it. Then, upload a scanned or photographed digital copy at the bottom of this page.
